Itemised Bill Request

To request an itemised bill free of charge for your registered line, visit any Vodafone store with your ID card or passport.

Some forms require a Customer Code or Account Reference number. This can be found on the first page of any bill related to that line or service.

    Prepaid

  • To transfer your Prepaid line to another person, fill in this form and visit any Vodafone store. Both you and the new owner must be present with your ID cards or Passports.

    Notarial Declaration

  • To transfer or disconnect a deceased person’s line, present this form signed by your notary together with the death certificate, and your ID card at any Vodafone store. If you want to take ownership of the line, also fill in the Transfer of Ownership form (prepaid or Pay Monthly, as relevant) found on this page.
